Enrollment here at St. Matthew begins with a meeting with our director and a tour of our facility. During the tour we will discuss your family's needs and desires for your child's early education experience. You will have a chance to meet our staff, see our classrooms, and have any questions that you may have answered.

Upon enrollment you will be asked to fill out all required paperwork, and pay a non-refundable registration fee. You will receive a parent handbook with all of our policies and procedures.

  • Child Registration Form

    This is our registration form. This tells us if you are looking for full time or part time care and what hours you will need care. 


  • Child Intake Form

    This form lets us get to know your child on a more personal level, and gives us an idea as to what you expect from us and if you have any concerns. 


  • Health Appraisal

    The Health Appraisal form must be filled out and signed by a physician before your child can come to school. All children must be up to date on immunizations or have an approved health department waiver to attend classes. 


  • Internet/photo Permission

    The internet/photo permission gives us permission to take your child's photograph while they are in our care and to post the photo in the classroom or on this web page and facebook page. Please note that we do not use names or any identifying information with your child's photo.
